Erin Kray, MA, ATR, LPAT, LPCC-S, has over over 17 years of experience specializing in diagnosis & mental health and substance use assessments. She is a Registered Art Therapist, Licensed Professional Art Therapist, and Licensed Professional Clinical Counselor with a Supervision Designation. She has a background in assessment & diagnosis, homeless services, compliance, supervision, and clay-based art therapy.

Clinical Assessment & Diagnosis

An assessment is an in-depth interview regarding a person's mental health or history of substance use. Evidence based self report tools can assist to supplement the detailed interview. This allows us to provide the most accurate diagnosis and recommendations that will best serve the client. Results of the assessment are shared with the client in the form of an assessment report as well as recommendations and referrals to appropriate treatment options in the community. Assessments are confidential.

Some people need fast and accessible substance use assessments because of a recent OVI/DUI arrest. Others are interested in hearing from another trusted professional if their mental health diagnosis is accurate. 

Initial consultations are free. Assessments/evaluations are by appointment only.

Initial Consultation

We start with a 15 minute phone call to collect pertinent information related to fit for type of assessment and share information about the assessment process. At this time we will schedule the virtual or in-home assessment. Schedule via booking link or feel free to email for more information erin@clevelandassessment.com.

Diagnostic Evaluation

A 90-180 minute interview will be conducted gathering presenting problem, medical/mental health/social/substance use histories. The interview includes screening for diagnoses, AOD or MH depending on assessment. Self-report inventories may be implemented for additional collateral information. Interviews with third parties maybe conducted for additional collateral information.

Referrals & Recommendations

Our findings are synthesized into a detailed clinical report, including specific recommendations for treatment, resources, and appropriate community referrals.
